We are happy you are here! You will find important information here regarding your first appointment with Dr. Carrigan. At your first visit, Dr. Carrigan will review your medical history, discuss your wants and areas of concern, explain your treatment, and answer any of your questions you may have. Below you will find the new patient forms. To save time you can print and fill out the forms prior to your visit, or you can arrive 10 minutes prior to your first appointment and fill them out in the office.
